Terms & Conditions – Account Change Request

I/We agree that changes on this form amend the previously signed Membership form, and I/we are subject to all terms and conditions of membership in Element Federal Credit Union.

If I am requesting to remove a joint account owner, I understand removal of a joint owner requires consent of all account owners. We will hold the Element Federal Credit Union harmless for actions regarding access. The removed owner relinquishes ownership interest including any membership share in the account(s). This relinquishment does not affect my/our obligation on any loan account(s).  I understand removal of a joint owner requires a notary signature, and this electronic form does not allow for a notary signature.
